Completing your check-in before you arrive saves time in the waiting room and helps us prepare for your visit.
1. Check in through healow
Download the free healow app or visit healow.com, then enter our practice code:

2. Complete your patient forms
Our patient forms are hosted on the Florida Digestive Health Specialists site. Please fill them out and bring them with you, or complete them through healow.
3. Bring these with you
- Your insurance card(s) and a photo ID
- A current list of your medications, including doses
- Any referral or authorization your plan requires
- Recent records or imaging related to your GI concern, if you have them
Preparing for Your Procedure
Following your prep instructions carefully is the single most important thing you can do for a successful procedure. If anything is unclear, call us at 813-600-3709 before your prep begins.
Colonoscopy
The most effective screening we have for colorectal cancer, and the only one that can remove polyps during the exam. Find the page that matches the prep your physician prescribed.
Upper Endoscopy
A short procedure that lets your physician examine the esophagus, stomach, and upper small intestine. The same instructions cover EGD, endoscopic ultrasound (EUS), and ERCP.
Billing and Insurance
Pay your balance online through our secure portal, or review the plans we participate with. Billing questions can also go to our office at 813-600-3709.
